Including the following details in the request will help to speed up the case resolution:
1. Full Name
2. Device's brand and model
3. IMEI code (press *#06# on your phone to retrieve it)
4. Mobile number with country code (a valid simcard needs to be inside the phone to receive the SMS command)
5. Proof of purchase/Receipt of the device (please take a picture of the phone's original bill and attach it on the form)
